Contact Your Dentist After Hours: For emergencies, call your local office.

Submit an on-line Grievance Form: Sent though our secure server directly to Newport Dental Plan. Or, print a PDF-format form.

The Department of Managed Health Care is responsible for regulating health care service plans. If you need help with a grievance involving an emergency, a grievance that has not been satisfactorily resolved by your plan, or a grievance that has remained unresolved for more than 30 days.
